Several years ago I created an All About My Dad questionnaire for my kids to fill out and give dad.
This year with a two and three year old I decided it was time to make a simpler page perfect for toddlers and preschoolers. (young kids can still use the other pages too!)

The All About My Daddy questionnaire has fewer questions and a bigger drawing area perfect for young kids.
My two year old didn’t really answer most of the questions but my three year old loved telling me the answers so I could write them on his paper.

Both of the kids loved drawing a picture for dad in the box.
After the kids are done coloring and answering the questions you can laminate the page so it will last as a fun keepsake to have for years to come.

I recommend printing this page on cardstock but you can print it on regular paper if you prefer.
