One of the “All About My Dad” printables is the perfect gift for kids to give their dads this Father’s Day (or his birthday!).

This free, downloadable template allows kids to answer fun questions about their dad’s interests, hobbies, and favorite things.

It’s a great way to get kids excited about gift-giving and to show dads just how much they are loved and appreciated. 

All About Dad Questionnaire

For the past couple of years, my kids have filled out a questionnaire about mom and dad each year. It’s so fun to see what their sweet answers are. I love it when young kids that don’t really understand age answer. 

Having kids answer the questions about their Dad can be so funny. There have been years where Dad is 3 years old just like they are and years where he is 70! (we got a good laugh form that one!)

The question sheet for this interview is cute and easy to fill out. If your child is too young to fill out the worksheet on their own you can always write down their answers.

Dads will love this personalized gift from their little ones and not only is it so much fun to read when they receive the gift, it’s also fun to look back and read as the kids grow older.

One of my favorite things to do is save these printable worksheets each year and see how the kids handwriting and answers change as they grow older.

How to Use the All About My Dad Questionnaire

The All About My Dad printable is a simple and easy-to-use template that’s perfect for kids of all ages. Have the kids answer questions about dad and after filling out the questionnaire they can personize the printable before giving it to dad.

The free father’s day questionnaire is a great activity to do at home with your kids or it can be a fun activity to do with a class at day care or kids Sunday school. 

Along with being a great addition or stand alone gift for dad’s birthday or father’s day this questionnaire is a great way for kids to work on basic writing skills.

Questions to Ask Kids About Dad

The free father’s day printable features a series of questions that kids can answer about their dad.

There are 3 different printables to choose from and the questions are slightly different on the different pages. Some of the questions include:

  • What is your dad’s name?
  • How old is your dad?
  • What is your dad’s favorite food?
  • What is your dad’s favorite color?
  • What is your dad’s favorite thing to do with you?
  • What time does your dad wake up?
  • What does your dad do at work?
  • What is your dad’s Job?
  • Why is your dad the best?
  • What is something your dad always says?
  • What is your dad’s favorite thing to do?
  • What is the funniest thing your dad does?

Kids can answer the questions in their own words and draw pictures to accompany their answers. There is also space to write the date and for the kids to write their name at the bottom.

What if My Child Can’t Write Yet?

Younger kids that aren’t yet writing simply ask them the question and write their answer.

I have gotten some funny answers from my kids when they still to young to write their own answers. Let younger kids color the questionnaire and draw a picture on the back after you help them with the questions. 

Older children can often do the printable page on their own without much help. The older kids will probably know more “correct” answers to many of the questions about dad but it’s still a fun way to wish dad a Happy Father’s Day. 

Tips for customizing the printable for your child and their relationship with Dad

While the All About My Dad printable is a great starting point, you can customize it to make it even more special for your child and their relationship with their dad. Here are a few tips to get you started:

  • Add additional questions that are specific to your child’s dad around the page or on the back. For example, if their dad loves music, you can ask them to name their dad’s favorite band or musician.
  • Encourage your child to draw pictures that reflect their dad’s interests and hobbies around the boarder of the worksheet. If their dad loves fishing, they can draw a picture of him holding a fishing rod.
  • Help your child write a special message to their dad at the bottom of the printable. This could be a message of love and appreciation or a funny joke that they know their dad will appreciate.
  • Help your child trace their hand (or foot) on the back of the questionnaire. Doing this each year you will be able to see how much your child has grown as their answers to the list of questions changes in future years. 

By customizing the printable, you can make it even more special and personalized for your child and their dad.

Creative Ways to Give the All About My Dad Printable

Last year we made a video of the kids answering each of these questions about dad as well as writing down the answers and it’s a priceless treasure.

I love being able to see the kid’s handwriting and drawings on the printed dad questionnaire and I love being able to hear their little voices and the unique way they say things on the video.

So grab your phone and ask your kids these questions. It’s a fun surprise for Dad to not only get the questionnaire but an adorable video too!

My Dad Questionnaire 

This dad sheet might be my favorite of the three options. It has questions for kids to answer just like the other pages and it has several fun ties color.

The best part is the frame with space for kids to color a picture of their dad (or paste a recent photo). I love seeing how kids draw their dad and I love it when they include themselves in the picture. 

All About Dad Printable

This version of the father’s day questionnaire has the heading “all about dad” in letter outlines so kids can color them.

The page also has an outline of a man for kids to be able to draw a face and color for their dad.
