I can’t believe it’s already time to start thinking about back to school. As you get ready for the new school year don’t forget to print out a copy of this first day of school questionnaire.
I’m a big fan of doing questionnaire printables with my kids. Not only are they a fun activity but also a great keepsake to add to their journals.
This first day of school questionnaire worksheet is a fun way to remember little details about your child and each school year as they grow up.
First Day of School Questionnaire
You can customize the grade and year on this first day of school printable so it can be used year after year, for any grade level.
There are several different sections to this printable with fun questions for your kids to answer.
The top section of the printable is the title “First day of” with an editable box for the grade level, school year, and date.
At the top of the page, there is also a box to add a first-day photo or draw a self-portrait whichever you prefer.
Below that is a space for your child to write their name. I love seeing how my kid’s handwriting changes as they grow up.
The next section is all about the details with space to write age, weight, and height.
Up next is a section for favorites. I wrote a few options (food, color, and book) in this section. You can edit the favorite questions to include whatever favorite items you want. There is space for 3 different lines.
The next box has space to write the name of your child’s teacher and their school.
Below that is a section about family. It includes lines to write how many brothers and sisters are in your family and what position your child is (oldest, middle, youngest).
The last line to fill in this questionnaire is what would you like to be when you grow up?
Don’t forget to put the filled-out first day of school questionnaire in a safe spot (I like to include it in my kid’s journals).
Now head over to my last day of school questionnaire and save it so you don’t forget to use it at the end of the year!
