Doing questionnaire printables with my kids is one of my favorite things. Not only are they a fun activity but also a great keepsake to add to their journals.

For a few years we have been doing this first day of school interview at the beginning of the school year. This year I decided I wanted to make one for the end of the school year, so we can see how much has changed in one year.
This last day of school survey / questionnaire worksheet is a fun way to remember little details about your child and each school year as they grow up.

Elementary school age kids and even those in middle school will love doing this last-day-of-school questionnaire so don’t forget to add it to your lesson plans for the last week of school. (you can get the instant digital download at the bottom of the post.)
Last Day of School Questionnaire (Editable)
This questionnaire is one of my favorite free printables I’ve shared recently. You can customize the grade and year on this last day of school printable so it can be used year after year, for any grade level.
There are several different sections to this printable with fun questions for your kids to answer.
The top section of the printable is the title “Last day of” with an editable box for the grade level, school year, and date.
At the top of the page, there is also a box to add last day of school pictures or draw a self-portrait whichever you prefer.

Below that is a space for your child to write their name. This is a great way to see how your kid’s handwriting changes and see your child’s name in their handwriting change as they grow up.
The next section is all about the details with space to write age, weight, and height.
Favorite Section
Up next is a section for favorites. I wrote a few options (favorite food, favorite color, and favorite book) in this section.
You can edit the favorite questions to include whatever favorite items you want. There is space for 3 different lines .of favorite things.

Here are a few ideas if you want to change this section:
- favorite subject
- favorite teacher
- favorite field trip
- favorite class party
- or anything else you want your child to share/remember
The next box has space to write your child’s favorite thing I learned about this year.
Family Section
Below favorites section is a section about family. It includes lines to write how many brothers and sisters are in your family and what position your child is (oldest, middle, youngest).
The last line to fill in this questionnaire is what would you like to be when you grow up?

Don’t forget to put the filled-out last day of school questionnaire in a safe spot (I like to include it in my kid’s journals/memory book as a super fun keepsake).

Additional Questions
This end of the year questionnaire is a fun last day of school activity and the perfect keepsake for kids.
If you want to encourage a little more writing I love to have kids write answers to different questions on the back…with more long form answers.

One of my favorite questions to have them answer is what there plans are for the summer (or what they are most looking forward to over summer break).
A few other fun questions you could ask are:
- what is the hardest thing you did this year?
- how have you changed since the first day of school?
- what is your favorite memory from this school year?
- if you could relive one day from this school year, which would it be and why?
- what are you most excited about in your next grade?
- if you could teach your own class next year, what would you teach?
